Part II of this two-part educational module seeks to improve transition and reentry to school and community for youth with disabilities in juvenile correctional facilities. Created by the Iris Center, Vanderbilt, and Claremont, the module addresses consideration for youth transition from juvenile correctional facilities back into the community, school, and workplace. It also provides specific recommendations for transition planning.
